Zusammenfassung

Die gewöhnlich durch den Stich von Schildzecken ausgelöste Lyme-Borreliose gilt sowohl bei uns in Europa wie auch in Nordamerika als die häufigste durch Arthropoden übertragene Infektionskrank heit [32]. Die möglichen Manifestationen der Er krankung sind außerordentlich vielfältig, und wie die Syphilis vermag die Lyme-Borreliose zahlreiche andere, nicht infektiöse und antibiotisch behandel bare Erkrankungen zu imitieren. Es kann daher schwierig oder unmöglich sein, eine Radikulopathie, eine Arthritis oder ein schweres chronisches Erschöpfungssyndrom ohne labordiagnostische Hilfs mittel einer Lyme-Borreliose zuzuordnen oder diese auszuschließen. Als Dermatologen sind wir in der glücklichen Lage, die wesentlichen Hautmanifesta tionen wie das Erythema migrans (EM) als das Leit symptom der Frühphase, die Lymphadenosis cutis benigna (Borrelien-Lymphozytom) des Stadiums 2 und die Acrodermatitis chronica atrophicans (ACA) als Spätmanifestation meistens schon durch den kli nisch-morphologischen Aspekt, ergänzt beim Lymphozytom und der Acrodermatitis durch den histologischen Befund, einigermaßen sicher diagnosti zieren zu können. In Zweifelsfällen müssen aber auch wir uns auf die zusätzlichen diagnostischen Hilfsmittel eines direkten oder indirekten Erreger nachweises stützen.
