Abstract
Until recently, the critical importance of the alveolar epithelial barrier in the pathogenesis and recovery from acute lung injury (ALI) has not been well appreciated. However, progress in three areas has created a breakthrough in our understanding of the importance of the alveolar epithelium in ALI. First, in the past ten years the mechanisms that regulate transport of fluid and protein across the normal alveolar epithelial barrier have been identified. Second, there is some new data on the mechanisms responsible for injury to the lung epithelial barriers. Third, because of our new understanding of fluid and protein transport across the normal alveolar epithelial barrier as well as new insights into the process of alveolar epithelial cell repair, it is now possible to study the resolution and recovery from ALI in both the experimental and clinical setting.
