Zusammenfassung
Die Anastomoseninsuffizienz ist eine der Hauptursachen für Morbidität und Mortalität nach Gastrektomie. Die Pathogenese der ösophagointestinalen Anastomoseninsuffizienz ist nur unvollständig erforscht. Die immer technisch bedingte eingeschränkte Blutzufuhr und lokale mikrozirkulatorische Störungen, die zu Nekrosen führen [10] sowie Fremdkörperreaktionen [5, 6] sind anerkannte Ursachen der Anastomoseninsuffizienz seit mehr als 100 Jahren. Die Anastomoseninsuffizienz ist jedoch eine septische Erkrankung und exogene sowie endogene potentiell pathogene Mikroorganismen, die den Verdauungstrakt besiedeln, spielen möglicherweise zusätzlich zu den Mikrozirkulationsstörungen in der Pathogenese eine ursächliche Rolle.
