Zusammenfassung
Die Technik der Kompressionsanastomose hat mit der AKA-II-Methode der Kompression mit nicht-resorbierbaren Kunststoffringen und der BAR-Methode — der Kompression mit resorbierbarem Material — Eingang in verschiedene Kliniken gefunden und gehört dort zum Standardverfahren. Das Prinzip der AKA-II-Kompressionsanastomose besteht darin, daß 2 Plastikringe im Darmlumen die Darmresektionsränder komprimieren. Die Ringe mit einem schmalen Saum komprimierten Gewebes lösen sich ab dem 4. Tag von der Darmwand ab, so daß eine fremdkörperlose Anastomose entsteht. Der Einführung der AKA-II-Technik in die eigene Klinik gingen tierexperimentelle Untersuchungen voraus, die zeigten, daß eine nahtlose fremdkörperlose Anastomose einer konventionellen Anastomose hinsichtlich der Morphologie, der Anastomosenheilung und biomechanischer Parameter überlegen war [10]. Die klinische Anwendung erfolgt im Rahmen einer prospektiven Studie, deren Untersuchungsergebnisse hier wiedergegeben werden und die eine Fortsetzung klinischer Anwendung an 140 Patienten [9] darstellt, so daß nun die eigenen Erfahrungen mit dieser Technik auf der Anwendung bei 348 Patienten beruhen.
