Zusammenfassung
Aspergillusarten sind ubiquitär vorkommende Schimmelpilze. Der Haupterreger einer Lungenaspergillose ist Aspergillus fumigatus [15]. Nach Inhalation der Pilzsporen und Besiedelung des Respirationstraktes werden diese von Alveolarmakrophagen phagozytiert oder durch neutrophile Granulozyten und Monozyten abgewehrt. Bei Immundefizienz sind diese Mechanismen eingeschränkt, so daß die Pilzsporen pathogen werden können und sich als systemische Organmykose manifestieren [3, 13]. So stellte die Aspergillose bei HIV-Patienten im Spätstadium der Erkrankung eine seltene, jedoch lebensbedrohliche Komplikation dar [1, 3, 7, 9, 12]. Hauptlokalisation einer Aspergillose ist die Lunge. Prädisponierende Faktoren für eine invasive Lungenaspergillose sind eine Granulozytopenie, eine vorausgegangene Steroid-, eine langandauernde Antibiotikatherapie oder eine Lungenvorschädigung wie z. B. durch eine CMV- oder Pneumozystis-carinii-Pneumonie. Nach Erstdiagnose beträgt die durchschnittliche Überlebenszeit meist nur 4 Monate [3].
