Zusammenfassung
Die Grundlage für die in diesem Kapitel dargestellten Belastungstests lieferten Leary und Allen [1]. Sie stellten 1941 fest, daß Patienten mit peripherer arterieller Verschlußkrankheit (PAVK), die in Ruhe palpable Fußpulse aufwiesen, nach Beinarbeit transient abgeschwächte oder gar völlig fehlende Fußpulse hatten. Sie interpretierten dieses Phänomen als Folge von hypoxisch bedingten peripheren Arterienspasmen. Andere Autoren meinten, das Phänomen sei Ausdruck einer Umverteilung von Blut in das Gefäßbett mit dem geringsten Widerstand, die Wadenmuskulatur. Die Richtigkeit der zweiten Theorie bestätigten Ejrup [2], Strandness und Bell [3] sowie Winsor et al. [4]. Diese Autoren konnten zeigen, daß signifikante Strombahnhindernisse proximal der Gefäße, welche die Wadenmuskulatur versorgen (Aa. surales), unter Beinarbeit immer zu passagerem Druckabfall im Bereich der Knöchelarterien führen, während Strombahnhindernisse distal des Abganges der Aa. surales dies nur ausnahmsweise verursachen. Zudem ergab sich aus diesen Arbeiten, daß der Abfall des Knöchelarteriendrucks als objektiver Parameter zur Beurteilung des Schweregrades und zur Verlaufsbeobachtung einer PAVK verwendet werden kann.
