Zusammenfassung
Die Diagnose der Lungenembolie (LE) ist oft schwierig. Einerseits sind die Symptome und die klinischen Zeichen zuwenig sensitiv und zu unspezifisch, andererseits ist die Referenzmethode, die Pulmonalisangiographie, mit einer gewissen Morbidität (Kontrastmittelallergie, Herzrhythmusstörungen, Gefäßverletzung, hämodynamische Entgleisung) behaftet. Überdies ist sie für den Patienten unangenehm und kostenintensiv. Diese invasive Technik kann also nicht als Screeningmethode angesehen werden. Aus diesem Grund wurde in den letzten 20 Jahren verschiedene andere Methoden angeboten, insbesondere die Ventilations-Perfusions-Szintigraphie und neuerdings die Bestimmung von Markern der Gerinnungs- und Fibrinolyseaktivierung im Plasma, wie den D-Dimeren.
