Zusammenfassung
Ovarialkarzinome haben eine schlechte Prognose. In den vergangenen 10–15 Jahren wurde schrittweise ein Behandlungskonzept entwickelt, dessen derzeitige Eckpfeiler die standardisierte prä- und intraoperative Diagnostik, die stadienangepaßte chirurgische Therapie sowie die primäre Chemotherapie mit platinhaltigen Therapieregimen sind. Die Behandlungsergebnisse zahlreicher, großer klinischer Studien weisen aus, daß bei fortgeschrittener Erkrankung (Stadium III und IV nach FIGO) unter kontrollierten Bedingungen im Rahmen einer konsequent durchgeführten Primärtherapie Ansprechraten zwischen 50% und 80% erreicht werden können (Omura et al. 1989; Meerpohl et al. 1991; Advanced Ovarian Cancer Trialist Group 1991). Die Stabilisierung der initial hohen Remissionsraten über längere Zeiträume gelingt allerdings nach wie vor zu selten, so daß bei Patientinnen mit fortgeschrittener Erkrankung die Chance 5 Jahre zu überleben derzeit mit etwa 15–25% angenommen werden muß. Die Notwendigkeit, nach weiteren Verbesserungen der Therapie zu suchen ist damit evident.
