Zusammenfassung

In den westlichen Industrienationen steht die koronare Herzkrankheit in der Morbiditäts- und Mortalitätsstatistik an erster Stelle. In Deutschland erleiden jährlich etwa 250000 Menschen einen akuten Herzinfarkt, der in etwa 35% tödlich verläuft. Weiterhin konnte gezeigt werden, daß Patienten mit einem linksventrikulären endsystolischen Volumen von >130 ml und einer linksventrikulären Auswurffraktion von < 40% eine deutlich geringere Fünfjahresüberlebensrate zeigen als Patienten mit einem endsystolischen Volumen von <95 ml und einer Auswurffraktion von >50% [48]. Hieraus ergibt sich, daß die Therapie der koronaren Herzkrankheit und insbesondere des akuten Myokardinfarkts darauf ausgerichtet sein muß, möglichst früh die zugrundeliegende Ursache, d. h. die Einengung des Lumens der Koronararterie bzw. den akuten Koronararterienverschluß, zu beseitigen, um eine Verbesserung der Symptomatik der Patienten, eine Reduktion der Infarktgröße und damit auch eine Verbesserung der linksventrikulären Funktion sowie eine Senkung der Mortalität zu erreichen. In der vorliegenden Übersichtsarbeit sollen neue Aspekte der Ursachen, Auswirkungen und Therapie der Myokardischämie dargestellt werden.
