Zusammenfassung
Gastrointestinale Lymphome leiten sich von Subpopulationen der B- oder T-Zell-reihe her, die Differenzierungsmerkmale des ortsständigen Immunsystems aufweisen. Sie zeichnen sich durch ein charakteristisches „Homing“-Verhalten aus, dessen Muster eine enge topographische Beziehung zur Mukosa erkennen läßt. Diese nach den bisher gängigen Klassifikationen nur unbefriedigend einzuordnenden Entitäten unterscheiden sich nach morphologischen, immunphänotypischen, klinischen und auch pathogenetischen Gesichtspunkten von nodalen Lymphomen [3]. Eine Besonderheit ist ihre häufig beobachtete Assoziation mit prälymphomatösen Läsionen, wie z. B. der Helicobacter pylori bedingten chronischen follikulären Gastritis [16], der Thyreoditis Hashimoto [5], der myopepithelialen Sialoadenitis [6] sowie der glutensensitiven Enteropathie bzw. nichttropischen Sprue und der ulzerösen Jejunitis [4, 9, 15, 17]. Bei diesen Krankheitsbildem dürfte eine persistierende Antigenstimulation zur genetischen Alteration lymphatischer Zellen mit Auftreten und Expansion eines malignen B- oder T-Zellklons führen. Bemerkenswert ist, daß frühe neoplastische Läsionen durch lokale Regulationsmechanismen, bei denen v. a. inflammatorischen Zytokinen eine Bedeutung zukommt, beeinflußbar und zumindest in einzelnen Fällen nach Therapie der zugrunde liegenden entzündlichen Veränderung reversibel sein dürften.
