Zusammenfassung
Die arterielle Hypertonie spielt eine wichtige Rolle für die Langzeitprognose von Nierentransplantaten. Die Inzidenz wird kurz nach der Transplantation mit bis zu 85% angegeben (De Strihou et al. 1983; Kasiske 1987), um danach langsam auf etwa 50% abzunehmen. Hill et al. (1991) konnten in einer Untersuchung der Todesursachen nach Nierentransplantation innerhalb der Jahre 1966–1987 zeigen, daß tödliche systemische Infektionen deutlich rückläufig waren, daß aber kardiovaskuläre Erkrankungen weiterhin in gleicher Häufigkeit für den Tod nierentransplantierter Patienten verantwortlich waren. Die arterielle Hypertonie muß dabei als ein entscheidender Risikofaktor gewertet werden. Sie läßt sich bei diesen Patienten auf unterschiedliche Ursachen zurückführen, die dem Transplantat, dem Empfänger oder aber der Therapie nach Transplantation zuzuschreiben sind (Tabelle 1).
