Lokale Therapie des Rektumkarzinoms

Zusammenfassung

Theoretisch kann man bei allen Karzinomen des mittleren und unteren Rektums, deren Lymphknoten nicht von Tumor befallen sind, die chirurgische Therapie durch lokale Exzision als heilend erachten. Hierbei handelt es sich um Tumoren des UICC-Stadiums I und II, d.h. um ca. 50% der Rektumkarzinome [1]. Auch die Karzinome des UICC-Stadiums IV bieten sich zur lokalen, aber palliativen Behandlung an, während das UICC-Stadium III nur die konventionelle chirurgische Therapie im Sinne der radikalen Resektion als sinnvoll erscheinen läßt. Allerdings tritt etwa bei der Hälfte dieser Neoplasien ein Tumorrückfall ein, so daß dieses Stadium nur bei einem Teil der Patienten durch die traditionelle chirurgische Therapie geheilt werden kann [2].
