Zusammenfassung
Die Technik der transurethralen Prostataresektion (TURP) wurde in den 30er Jahren entwickelt. Mehr als ein Jahrzehnt später wurden zum erstenmal kardiovaskuläre und zentralnervöse Komplikationen beschrieben, die auf die Absorption der während des Eingriffs notwendigen Spülflüssigkeit zurückzuführen waren. Im Jahr 1947 berichtete Creevy [2] über hämolytische Reaktionen, die infolge intravasaler Resorption des damals noch als Irrigationsmedium verwendeten destillierten Wassers auftraten. Dies führte sehr bald zur Benutzung isotoner, unter Verwendung von Harnstoff, Glukose oder Mannit hergestellter, nichthämolysierender Spülflüssigkeiten, die ihrerseits jedoch erhebliche Nebenwirkungen aufwiesen [18]. Heute werden überwiegend Zuckerlösungen als halbisoosmolare Lösungen verwendet, z.B. „Puri-sole SM verdünnt“ (Fresenius), das pro Liter Spülflüssigkeit 27,0 g Sorbit und 5,4 g Mannit enthält. Andere Autoren, v.a. aus dem angelsächsischen und skandinavischen Raum, empfehlen alternativ auch eine 2,2%ige Glycinlösung [8]. Höhere Serumkonzentrationen von Glycin haben allerdings ebenfalls erhebliche Nebenwirkungen zur Folge (Sehstörungen bis zur passageren Blindheit, andere zerebrale Störungen).
