Zusammenfassung
Die Idee„ die zur Entwicklung der Kehlkopfmaske (KM) führte, entstand aus Überlegungen konstruktiver, anatomischer und physiologischer Natur [1, 2, 3, 5, 12, 17, 19]. Der Konstrukteur, der mit der Aufgabe befaßt ist, 2 Röhrensysteme wie die Trachea und einen zuführenden Gasschlauch miteinander zu verbinden, wird eine End-zu-End-Positionierung mit einem entsprechenden Verschluß dem Ineinanderschieben von 2 Röhren vorziehen. Letzteres entspricht der Intubation, die prinzipiell den Nachteil hat, den Durchmesser des Luftweges zu verringern und die spezielle Motorik der oberen Luftwege zu behindern. Kehlkopf und Trachea sind ausschließlich dafür konstruiert, Gaspartikel aufzunehmen, die Luftwege für feste Partikel der Nahrungsaufnahme zu verschließen und die Stimmbildung zu ver-möglichen. Cuff und Körper des Trachealtubus drücken gegen eine hochspezialisierte Schleimhaut und unterdrücken ihre Funktion [1, 2, 3, 12]. Von diesen Überlegungen ausgehend ist der Trachealtubus trotz aller seiner Vorteile so unphysiologisch, wie die Gesichtsmaske unpraktisch ist. Bei ihr sind beide Hände des Anästhesisten gebunden, auch wenn aus der Situation der Narkose her dringende andere Verrichtungen erforderlich wären.
