Zusammenfassung
Spontan erworbene Inhibitoren bzw. Antikörper gegen einen Gerinnungsfaktor bei vorher gerinnungsnormalen Personen sind eine seltene Ursache für erworbene hämorrhagische Diathesen. Am häufigsten werden Autoantikörper gegen Faktor VIII:C (Autoimmunhemmkörperhämophilie) beobachtet. Die Häufigkeit wird auf 0,2-1 pro 1 Mio. Einwohner und Jahr geschätzt [1]. Die Erkrankung ist mit einem hohen Blutungsrisiko belastet. In einer großen Übersicht von 215 Patienten über einen Zeitraum von 10 Jahren fanden Green u. Lechner [21 in 87% der Fälle schwere Blutungen, und 22% der Patienten verstarben infolge der hämorrhagischen Diathese. Dies zeigt, daß die Behandlung von Blutungskomplikationen ein erhebliches Problem sein kann. Feste Regeln zur Inhibitortherapie bestehen nicht. Wir berichten hier über den Verlauf und die Behandlung von 3 Patientinnen, 2 mit einem hohen und 1 mit einem niedrigen Anti-Faktor-VIII:C-Titer.
