Zusammenfassung
Mit den Fortschritten in der Transplantationsmedizin ist der Bedarf nach Organen im Verhältnis zu deren Verfügbarkeit überproportional gewachsen In der Diskussion über Kriterien für die Vergabe dieses “knappen Guts” wird in letzter Zeit zunehmend der Begriff Lebensqualität genannt. Die Frage, ob die Lebensqualität der “Transplantationskandidaten” in der Zuweisung von Organen eine Rolle spielen sollte, führt zu unterschiedlichsten Reaktionen. Zum einen eine vehemente Ablehnung, gepaart mit Assoziationen zum gläsernen Menschen, Orwell’schen Verhältnissen und sozialdarwinistischen Denkmustern; zum anderen eine pragmatische Zustimmung, gepaart mit einer Orientierung am maximal zu erwartenden Nutzen, der Ansicht, daß die Lebensqualität implizit sowieso in die Indikationsstellung einfließe, und dem Bedürfnis nach einer transparenten Entscheidungsfindung. Die häufigste Reaktion ist allerdings eine skeptische Nachfrage danach, was unter Lebensqualität zu verstehen sei, inwiefern sie sich überhaupt als Allokationskriterium eignen könne und auf welcher empirischen Basis die Debatte über “Pro” und “Kontra” beruhe. Dieser letztgenannte Zugang zum Problem liegt den folgenden Ausführungen zugrunde.
