Zusammenfassung
Die Beantwortung der Frage, welchen Einfluß das Alter auf die Häufigkeit und die Therapie des Prostatakarzinoms einnimmt, ist für ein Screening-Programm von großer Bedeutung. Liegt ein direkter Zusammenhang zwischen dem Alter und der Häufigkeit der Erkrankung vor, kann möglicherweise eine Altersgruppe definiert werden, bei der eine allgemeine Vorsorgeuntersuchung besonders sinnvoll ist. Bei Überprüfung der Wertigkeit eines Screening-Programmes muß auch untersucht werden, ob sich für die diagnostizierten Patienten mit einem Prostatakarzinom eine günstige Therapieoption ergibt und inwieweit diese vom Lebensalter beeinflußt wird. Bei dem bekannt langsamen Tumorwachstums des Prostatakarzinom bedeutet dies z.B., daß für die Patienten eine aggressive Therapie nur dann angezeigt ist, wenn sie aufgrund ihrer Lebenserwartung davon profitieren. Dabei ist jedoch zu beachten, daß die durchschnittliche Lebenserwartung in Deutschland zunimmt — so ist die Lebenserwartung der 60jährigen Männer in der BRD zwischen 1972 und 1988 um über 2 Jahre angestiegen.
