Zusammenfassung
Das Prostatakarzinom tritt in einer Vielzahl von Manifestationsformen auf, und es ist bekannt, daß die Charakteristika des Tumors und des Wirts in stärkerem Maße den Verlauf der Krankheit bestimmen als die Therapie. Tatsächlich beeinflußt in nicht wenigen Studien die Präselektion der Patienten das Endergebnis deutlicher als die therapeutische Modalität. Diese Feststellung läßt sich an folgenden Beispielen illustrieren:

In der Vergangenheit wurde in der EORTC-Studie 30853 ein Vorteil der kompletten Androgendeprivation bei der Behandlung des fortgeschrittenen Prostatakarzinoms im Vergleich zur partiellen Androgendeprivation belegt (Denis et al. 1990). Entsprechend dieser Studie, die eine randomisierte Behandlung zwischen Orchiektomie und Goserelin-Acetat (Zoladex) und Flutamid (Fugerei) vorsieht, zeigt sich, gemessen am Studienendpunkt Leistungsindex, eine Überlegenheit der Kombinationsbehandlung (Abb. 1).
