Das Morbiditätsspektrum bei Patienten mit Schlafapnoe und nächtlichen bradykarden Herzrhythmusstörungen

Zusammenfassung

Über die Inzidenz bradykarder Herzrhythmusstörungen im Schlaf existieren bislang nur vereinzelte Untersuchungen. Erst mit der Einführung des Holter-EKG in die Routinediagnostik kardialer Arrhythmien konnte aufgezeigt werden, daß es auch im Schlaf zu passageren Störungen des Herzrhythmus kommen kann, die einer differenzierten Bewertung bedürfen. Das Herzfrequenzverhalten, das einer reflektorischen und zentralnervösen Steuerung unterworfen ist, wird auf den Antagonismus parasympathischer und sympathischer Aktivität zurückgeführt, wobei im Schlaf der vagale Einfluß dominiert. Daß ähnlich wie bei den tachykarden Arrhythmien die Altersabhängigkeit das Auftreten bradykarder Herzrhythmusveränderungen bzw. -störungen beeinflußt, konnte durch zahlreiche Untersuchungen bestätigt werden [3–6, 11, 24].
