Zusammenfassung

Prognostisch relevante Faktoren sind beim papillären Schilddrüsenkarzinom aufgrund umfangreicher retrospektiver Analysen amerikanischer Autoren definiert worden. Sie haben auch in Deutschland zu einer Befürwortung eines eingeschränkt radikalen operativen Vorgehens bei kleinen (<1,5cm) solitären Tumoren und beim Fehlen von Lymphknoten oder Fernmetastasen geführt [10–14, 16, 18–23, 25, 26, 29–46]. Auf der anderen Scite konnten wir im eigenen Krankengut für das follikuläre Schilddrüsenkarzinom nachweisen, daß die von amerikanischen Autoren festgestellten prognostisch relevanten Kriterien nicht uneingeschränkt auf unser Krankengut übertragbar sind. Vor allem besteht eine erhebliche Diskrepanz zwischen dem in der Regel blanden Verlauf in den USA und dem in nahezu einem Viertel der Fälle zu beobachtenden tödlichen Ausgang ausgedehnt invasiv wachsender papillärer Karzinome in Europa, ohne daß es hierfür eine Erklärung gibt [2]. Darüber hinaus kann für jeden einzelnen Patienten die Beurteilung der individuellen Prognose aufgrund der großen Variabilität des biologischen Tumorverhaltens problematisch Scin. Die Ermittlung weiterer prognostisch relevanter Faktoren ist hierfür notwendig.
