Zusammenfassung
Die Sarkoidose (M. Boeck, benigne Lymphogranulomatose) ist als Systemerkrankung durch diffuse Granulombildungen charakterisiert und führt bei unbekannter Ätiologie (Jones u. Davies 1980) vom respiratorischen Bereich ausgehend zum Befall von Lymphknoten, Haut, Augen und aller inneren Organe. Eine Granulomatose des Knochenmarkes ist bei 3–36% der Patienten (Israel u. Sones 1958; Uehlinger u. Wurm 1976) zu beobachten und stellt in der Regel eine Spätmanifestation der chronischen Verlaufsform der Sarkoidose dar (Löfgren 1953). Es entwickeln sich meist multifokale reaktive Osteosklerosen oder Osteolysen mit bevorzugter Lokalisation in den kurzen Röhrenknochen von Fingern und Zehen (Uehlinger u. Wurm 1976), wobei es nicht zum Einbrechen in benachbarte Gelenke und anliegende Haut oder zur Sequestrierung kommt (Jüngling 1920, Nielsen 1934). Neben Lungenfunktionsprüfung, Tuberkulintest, Calcium- und ACE-Bestimmung sowie Szintigraphie erfolgt die Diagnosesicherung histologisch (Nakhosteen u. Maassen 1981).
