Dermatitis herpetiformis — eine Systemkrankheit?

Zusammenfassung

Nach einem Jahrhundert klassifikatorischer Diskussion ist die DH durch drei Kriterien bestimmt [9]: klinisch durch heftig brennend-juckende Papulovesikel vorwiegend der Extremitätenstreckseiten; histologisch durch neutrophile Infiltrate der Papillen-spitzen und subepidermale multilokuläre Blasenbildung; in der Immunfluoreszenz durch granuläre IgA-Niederschläge in den dermalen Papillen. Weitere Merkmale sind ihr benigner wenn auch beschwerlicher Charakter, ihr seltenes Vorkommen (genaue Inzidenz unbekannt), ihre Androtrophie (Männer doppelt so häufig befallen wie Frauen), ihr schubhafter Verlauf und ihre hohe Assoziation mit glutensensitiver Enterophathie (GSE).
