Blutungen bei Verletzungen des Harntrakts

Summary

In 10% of the patients with blunt abdominal trauma injuries of the urinary tract can be found. Hematuria is the most common symptom which is present in about 80–90% of these cases. Absence of hematuria, however, does not preclude urinary tract injury. Other symptoms such as perineal or scrotal hematoma as well as bleeding from the urethral meatus are additional signs that have to be carefully looked for, especially in the unconscious patient. Excretory urography is the most important diagnostic procedure.

Management of patients with a polytrauma should be a multidisciplinary approach including the early attendance of an urologist if urinary tract injury is suspected.

Zusammenfassung

Bei 10% der Patienten mit stumpfem Bauchtrauma liegen Verletzungen des Harntrakts vor. Leitsymptom dieser Verletzungen ist die Hämaturie, die in 80–90% der Fälle nachweisbar ist. Das Fehlen einer Hämaturie schließt eine Verletzung des Harntrakts jedoch nicht aus. Nach weiteren Symptomen wie perinealen oder scrota-len Hämatomen bzw. nach einer Blutung aus der Urethra sollte insbesondere bei bewußt-losen Patienten gründlich gesucht werden. Das Ausscheidungsurogramm stellt die wichtigste diagnostische Maßnahme dar.

Die Behandlung polytraumatisierter Patienten muß interdisziplinär erfolgen, wobei ein Urologe frühzeitig hinzugezogen werden sollte, wenn eine Verletzung des Harntrakts möglich erscheint.
