Selbstexpandierende Stents in Ösophagus und Magen — eine Standortbestimmung

Summary

Between 8/91–2/93 a total of 18 self-expanding metal stents were implanted in 16 patients with obstructions of the esophagus or the gastric outlet. Following endoscopic examination and bougienage the stents were placed with a small lumen catheter. We treated 7 otherwise inoperable patients with a 14 mm Wallstent and 9 with a 18 mm Nitinol-Stent. Reobstruction occurred in 7 due to tumor infiltration. Recanalisation was achieved by laser or diathermia. Other complications were food impaction (3), bleeding (3), esophagobronchial fistula (1) and stent migration (2). We recommend metallic stents only in cases of failure with conventional prostheses.

Zusammenfassung

Im Zeitraum von 8/91–2/93 wurden 16 Patienten mit malignen Obstruktionen des Ösophagus und des Magenausgangs Stents unter endoskopischer Kontrolle implantiert. Wir behandelten 7 lokal inoperable Pat. mit einem 14-mm-Wallstent und 9 Pat. mit einem 18-mm-Ultraflex-Nitinol-Stent. Bei 7 Pat. kam es zu einer Reobstruktion durch Tumorinfiltration. Eine Rekanalisation wurde durch Laser oder Diathermie erreicht. Andere Komplikationen waren Bolusverschlüsse (3), Blutungen (3), ösophagobronchiale Fistel (1) und Stentdislokationen (2). Wir empfehlen die Anwendung von Metallstents nur beim Versagen der konventionellen Pertubation.
