Radiologische Planung und Darstellung der Charakteristika von Distraktionsosteotomien

Summary

In patients with posttraumatic leg length discrepancy of 2–4 cm, distraction corticotomy offers a good alternative to shortening osteotomy. Discrepancies over 3 cm should be treated by callotasis to restore the correct anatomical, cosmetic and functional symmetry including an equal level of the joints. The techniques of corticotomy and callotasis is well as the application of single sided frames have turned distraction osteotomies more convenient, less risky and more effective. The success is determined essentially by the quality of the precise preoperative clearing up of the pathological condition by comparative X-rays of the axis and the height of the joints, CT-scan, etc. The exact radiological follow-up is meaning for the postoperative radiological judgment. We demonstrated the results of 32 lengthening osteotomies applying a single-sided frame, characteristic signs of development of the regenerative zone with a central translucent zone replaced by the sclerosing zone followed by the zone of the compact bone. Developing disturbances in the distraction zone and their causes are demonstrated.

Zusammenfassung

Bei posttraumatischen Längenunterschieden der Beine zwischen 2–4 cm bietet die Distraktionsosteosynthese eine gute Alternative zur Verkürzungsosteotomie. Längendifferenzen über 3 cm sollten durch eine Kallotasis therapiert werden, um eine korrekte anatomische, kosmetische und funktionelle Symmetrie bei Höhengleichheit der Gelenke wiederherzustellen und dem Patienten ein lebenslanges Angewiesensein auf Schuherhöhungen und Prothesen zu ersparen. Die Techniken der Kortikotomie und Kallusdistraktion sowie die Anwendung unilateraler Fixationselemente haben die Distraktionsosteotomie einfacher, risikoärmer und effektiver gestaltet. Der Erfolg wird wesentlich von der Qualität der präzisen präoperativen Abklärung des pathologischen Zustandes durch Achsenaufnahmen, CT, Höhenvergleich der Gelenke etc. bestimmt. Für die postoperative radiologische Beurteilung ist die exakte radiologische Verlaufsbeurteilung von Bedeutung. Demonstriert werden die Ergebnisse von 32 Verlängerungs-osteotomien unter Verwendung eines unilateralen Fixateurs, die radiologischen Verlaufscharakteristika der Kallotasis in der Regeneratzone mit einer zentralen Transparenzder sich anschließenden Sklerose — und der Zone des kompakten Knochens. Sich entwikkelnde Störungen in der Distraktionszone und ihre Ursache werden demonstriert.
