Summary
ACL prothesis raises three major problems so far: 1. insufficient mechanical properties, 2. foreign body reaction and 3. insufficient anchorage in the bone. Generally, AO screw or staple fixations are used. Integration into the bone depends on prothetic structure and design. An animal trial (20 sheep, 8–64th week) using an Aramid prothesis showed that, with 2 mm holes, fingerlike bony integration leads to sufficient mechanical support (“Pull out” tests with Zwick testing device). Mechanical testing was correlated with histological and X-ray evaluation which both confirmed real osteointegration into the prothesis. These results suppose that appropriate ACL prothesis design allows early removal of fixation screws after 8 weeks.
Zusammenfassung
Kreuzbandprothesen haben bisher wegen drei wesentlicher Probleme nicht überzeugen können: Mechanische Eigenschaften, Fremdkörperreaktionen, Verankerung. Für die meisten Modelle sind Schrauben- oder Klammerfixationen vorgesehen. Eine tierexperimentelle Untersuchung am Schaf zeigt am Beispiel einer Aramid-Kreuz-bandprothese, daß es bei großporigem (02 mm) Prothesendesign zu einer Knochenneubildung durch die Prothese hindurch kommt. Histologisch und radiologisch ergibt sich, daß Knochen fingerförmig durch das Kunstband wächst. Nach Entfernen der primären Fixation reißt das Band im Bruchlastversuch nicht aus den Bohrkanälen aus, sondern durchschneidet die Kortikalis des Tibiaplateaus bis zum Ausriß. Fazit: bei entsprechendem Design wird eine frühe Schraubenentfernung (nach 8 Wochen) möglich.
