Respiratorisches Versagen

Summary

Respiratory failure following trauma is a result of locally accentuated systemic inflammatory response. Pharmacological intervention is limited. Mechanical ventilation is directed to alveolar recruitment without damaging the diseased lung in terms of barotrauma, volutrauma and O₂-toxicity. This can be done by pressure limitation, low tidal volumes, increased I: E ratio and intrinsic PEEP. The results using this type of respiratory management are encouraging. Extracorporal gas exchange is restricted to rare cases with severe ARDS. Prophylactic measures are adequate shock-therapy and early fracture stabilisation in “little” invasive techniques.

Zusammenfassung

Das respiratorische Versagen nach Trauma ist Folge einer lokal akzentuierten Entzündungsreaktion. Möglichkeiten einer pharmakologischen Intervention sind gering. Ziel der Respiratorbehandlung ist die Vergrößerung der Gasaustausch-fläche unter Vermeidung von Barotrauma, Volutrauma und O₂-Toxizität. Der Ansatz liegt dabei in Druckbegrenzung, kleinen Atemzugvolumina, verlängertem I:E Verhältnis und intrinsic PEEP. Die Ergebnisse mit differenzierten Beatmungstechniken sind vielversprechend. Der extrakorporale Gasaustausch bleibt seltenen Ausnahmeindikationen vorbehalten. Prophylaktische Maßnahmen umfassen eine adäquate Schocktherapie und frühestmögliche Osteosynthese in „gering” invasiver Technik.
