Summary

Macrohemodynamical approaches (e.g., determination of systolic ankle arterial pressures) are of limited value only, for the indication of distal vascular reconstruction with uncertain outcome or to select more homogeneous groups of patients for clinical trials in advanced stages of peripheral arterial occlusive disease since they do not reflect the true state of acral malperfusion. Thus, clinical microcirculatory research techniques have been proposed for the laboratory diagnosis of chronical critical leg ischaemia (CLI). Presently, only transcutaneous oxymetry seems to be suitable for practical application in CLI. Neither thermography nor laser-Doppler-fluxmetry will improve clinical diagnosis of CLI in the near future. Promising results obtained with intravital capillaroscopy and fluorescein perfusography still have to be confirmed by other authors.

Zusammenfassung

Die apparative Diagnostik der chronischen kritischen Extremitätenischämie soll u. a. die differentialtherapeutischen Entscheidungen zu prognostisch unsicheren cruralen Gefäßrekonstruktionen oder die Bildung homogener Kollektive für Therapiestudien in fortgeschrittenen AVK-Stadien unterstützen. Makrohämodynamische Untersuchungsparameter (z. B. systolische Knöchelarteriendrücke) liefern dabei nur bedingt zuverlässige Aussagen, da sie die akrale Hautdurchblutung nicht erfassen. Verfahren der klinischen Mikrozirkulationsforschung sollen diese Lücke schließen. Aufgrund der gegenwärtigen Erfahrungen kommt nur die transkutane Sauerstoffpartialdruckmessung für die Praxis in Frage. Weder die Thermographie noch die Laser-Doppler-Fluxmetrie dürften in absehbarer Zukunft einen klinischen Stellenwert erlangen. Einzelne positive Ergebnisse mit der intravitalen Kapillarmikroskopie und der Fluoreszeinperfusographie müssen vorerst noch durch andere Autoren bestätigt werden.
