Grenzen chirurgischen Handelns in der Kinderchirurgie

Summary

The extremely difficult choice between the “Sanctity of Life” and “Quality of Life” concepts is discussed. The author does not feel, that an uncompromising adherence to the former alternative without regard for ensuing burdens on the patient or his environment is justified. Irrespectively of the abhorring implications, even economical factors, global overpopulation and other collective aspects cannot be entirely disregarded. The duty of the pediatric surgeon to accept most of the responsibility in a decision to refrain from treatment in desolate cases is underlined as is the importance of securing a qualified, unbiased second opinion. On the other hand the institution of multiexpert boards to take over the non-treatment decisions is not recommended.

Zusammenfassung

Die Wahl zwischen Pflichtethik und Konsequenzethik wird diskutiert, wobei schmerzhafte Entscheidungen über Prioritäten getroffen werden müssen und nicht einmal eine Beachtung der ökonomischen Begrenzungen zu umgehen ist. Die Verantwortung des behandelnden Arztes in den entstehenden, stark gefühlsgeladenen Situationen wird besprochen sowie auch die Konflikte zwischen individueller Empathie und prinzipieller Logik. Die Zuziehung eines urteilsfähigen, unbefangenen Kollegen angesichts eines Verzichtes auf Therapie wird empfohlen. Dagegen wird das Weitergeben der Entscheidung an ein größeres Greminum abgelehnt.
