Zusammenfassung
Aus vielerlei Gründen nimmt das Nasopharynxkarzinom eine Sonderstellung unter den malignen Geschwülsten des Kopf- und Halsgebietes ein. So ist es beispielsweise der einzige Tumor in diesem Bereich, der aufgrund seiner besonderen anatomischen Lage keine sichere Seitendifferenzierung eine Körperhälfte betreffend zeigt. Weiterhin scheinen hinsichtlich der Karzinogenese die ansonsten bei den Kopf-Hals-Malignomen üblichen Risikofaktoren wie Zigarettenrauchen und Alkoholkonsum, oder vermehrte Expositionen gegenüber physikalischen Noxen wie beispielsweise ultravioletter Strahlung, keine Rolle zu spielen. Ganz im Gegensatz dazu scheint die Epidemiologie dieses bei uns seltenen Tumors dafür zu sprechen, daß rassisch-genetische Faktoren und wahrscheinlich auch eine Virus- ätiologie (Epstein Barr-Virus) von ätiopathogenetischer Bedeutung sind. Auch hinsichtlich der Klassifikation und des Tumorstaging hat sich im Gegensatz zu den übrigen Kopf- und Hals-Malignomen noch keine einheitliche Nomenklatur durchgesetzt. Hier stehen sich die überwiegend von der Auffassung der westlichen Länder geprägte AJC- und UICC-Klassifikation und die in den fernöstlichen Ländern mit teilweise extrem hoher Inzidenz gebräuchliche Ho-Klassifikation gegenüber. Hinsichtlich der histologischen Klassifikation sind mittlerweile nicht weniger als vier verschiedene Normen im Gebrauch. An unserer Klinik benutzen wir die „Kölner Variante“ des WHO-Schemas, die uns sowohl von der pathohi- stologischen Aussage als auch von der klinischen Relevanz her am meisten zusagt.
