Overview of Clinical 31P Magnetic Resonance Spectroscopy Investigations and Applications for Renal Transplant Viability Assessment

Phosphorus-31 magnetic resonance spectroscopy (31P MRS) is a nondestructive and noninvasive technique in which intracellular phosphorus metabolites are used as viability parameters for assessment of cadaveric renal viability pretransplantation and to delineate ischemic metabolic pathways for formulation of better organ preservation maneuvers. This overview summarizes the development of 31P MRS for the assessment of renal transplant viability in the setting of acute ischemia, with its current clinical applications. In addition, the use of 31P MRS for preservation studies and in situ assessment of renal transplant metabolism using surface coils is discussed.
