Zusammenfassung
Die häufigste präkanzeröse Läsion im Kolorektum ist das Adenoma. Seine makroskopische Form ist sehr unterschiedlich, zumeist präsentiert es sich als Polyp (gestielte oder breitbasige Läsion oberhalb der Schleimhautoberfläche); seltener ist das flache Adenom, das wenig erhaben oder leicht eingesenkt erscheint oder nur durch eine Diskoloration auffällt (solche „ebenen“ Adenome verdecken die submuköse Gefäßarchitektur). Alle Adenome gehen von den Krypten aus (Einzelkryptenadenome) und können Ausgangspunkt malignen Wachstums sein [11, 19]. Im allgemeinen bestehen Beziehungen zwischen der Adenommasse und der Tendenz zur malignen Entartung. Es gibt die Möglichkeit der Karzinomentstehung aus Adenomen ohne makroskopisch erkennbare „Läsion“ (also auch ohne Polypen), was nicht berechtigt von einem „De-novo-Karzinom“ zu sprechen. Die Kriterien eines „De-novo-Karzinoms“ wurden bisher nicht exakt definiert [11, 19].
