Mikrografische Chirurgie: Die Therapie, die dem lokalen Infiltrationsverhalten des Basalioms gerecht wird

Das Basaliom

Zusammenfassung

Infolge einer praktisch fehlenden Metastasierung stellt die Rezidivneigung das therapeutische Hauptproblem des Basalioms dar. Um die Suffizienz einer Therapie beurteilen zu können, muß man also die Rate der postoperativ auftretenden Rezidive heranziehen. Hier unterscheiden sich die Angaben in der Literatur sehr stark (von 0,3% bis 50%) [4, 6, 8, 9, 11, 12, 13, 20]. Die besten Ergebnisse (0,3–2,5%) weist die mikrografische Chirurgie auf. Diese ist definiert als schrittweise Exzision von Hauttumoren mit einer lückenlosen histologischen Darstellung der gesamten Exzisataußenseite (3-D-Histologie) bis zum Nachweis tumorfreier Schnittränder. Die histologische Aufarbeitung ist sowohl im Kryostatschnitt- als auch im Paraffinschnittverfahren möglich.
