Zusammenfassung
Die Prognose von Patienten mit einem Nierenkarzinom ist nach wie vor infaust. Erfolgversprechende Behandlungsstrategien existierten bislang weder für das vermeintlich kurativ operable noch für das bereits metasta-sierte Nierenkarzinom. Während 40% aller Patienten bereits bei Diagnosestellung Metastasen aufweisen und im statistischen Mittel 7–11 Monate überleben (STEVEN et al. 1977), versterben von den restlichen 60% der Patienten mit einem klinisch lokalisierten Nierenkarzinom trotz radikaler Tumornephrektomie bis zu 50% innerhalb von 5 Jahren an Metastasen des Primärtumors (ROBSON et al. 1968). Im eigenen Patientengut fanden wir eine Metastasierung innerhalb von 5 Jahren nach radikaler Nephrektomie bei Patienten mit einem Nierenkarzinom im Stadium I-III sogar in 80%, so daß die radikale Tumornephrektomie für den Großteil der Patienten auch mit lokalisierter Erkrankung nur ein palliativer Eingriff ist (OTTO et al. 1986). In Anbetracht dieser schlechten Prognose ist es verständlich, daß man nicht nur durch die Verbesserung der operativen Techniken, sondern auch durch systemische Therapieformen versucht hat, den natürlichen Krankheitsverlauf des Nierenkarzinoms positiv zu beeinflussen. In einer Vielzahl von klinischen Studien erwies sich das Nierenkarzinom jedoch als nahezu resistent gegenüber allen bekannten zytostatischen Therapieschemata (DE KERNION 1983).
