Zusammenfassung
Während schwere hämodynamische Reaktionen mit Absinken von totalem peripherem Widerstand und Ansteigen des pulmonalen Gefäßwiderstands nach der Antagonisierung einer systemischen Heparinisierung durch Bolusinjektion von Protamin wohlbekannt sind, gilt die Injektion von Heparin selbst vor der Implantation von Gefäßprothesen oder vor extrakorporaler Zirkulation weitgehend als risikolos. Anhand von Einzelfallbetrachtungen sowie bei genaueren Untersuchungen wird jedoch klar, daß unmittelbar nach i. v. Bolusinjektion von Heparin in Dosen von mehr als 10000 Einheiten eine deutliche hämodynamische Reaktion mit Absinken des arteriellen Mitteldrucks und des peripheren Gefäßwiderstands sowie Ansteigen des Herzindex auftritt [3, 7, 31]. Diese Reaktion kann in Einzelfällen erheblich ausgeprägt sein und den Patienten vital gefährden. Da unmittelbar nach Heparininjektion im Rahmen des operativen Vorgehens eine Reihe von chirurgischen Manipulationen wie etwa in der Herzchirurgie die Kanülierung von Aorta und rechtem Vorhof, in der Gefäß-chirurgie das Abklemmen von Gefäßen erfolgt, wird die heparinbedingte hämodynamische Reaktion jedoch meist diesen chirurgischen Manipulationen zugeschrieben und nicht im erforderlichen Ausmaß beachtet.
