Anästhesiologische Besonderheiten bei Kindern

Zusammenfassung

Im Kindesalter stellen kongenitale Naevi die häufigste Indikation für einen dermatologischen Eingriff dar, mit einer enormen Variabilität, was Größe und Lokalisation angeht. Es kann also, was Eingriff und Anaesthesie angeht, nicht standardmäßig vorgegangen werden. Die Art der Anästhesie muß entsprechend des Alters des Kindes, der Lokalisation, der Größe des Eingriffs und der Erfahrung des Operateurs sowie des Anästhesisten und auch der Kooperation zwischen beiden individuell bestimmt werden. Die Möglichkeiten reichen von der Allgemeinanästhesie in Intubation oder Spontanatmung über sog. Dualanästhesien, d.h. kurze Allgemeinanästhesie mit gleichzeitiger Leitungs- oder Infiltrationsanästhesie bis zur reinen Lokalanästhesie mit oder ohne Prämedikation. Bei allen diesen Anästhesieformen stehen Dosisprobleme in Abhängigkeit vom Körpergewicht im Vordergrund. In der Praxis genauso wichtig ist eine gute psychische Führung des Kindes, um die unvermeidliche Irritation vor dem unbekannten Eingriff zu mindern. Bei gutem Kontakt zum Kind können durchaus auch mittelgroße Eingriffe in Infiltrations- oder Leitungsanästhesie durchgeführt werden.

Congenital nevi are the most frequent indication for skin surgery in children. Due to the enormous variability in size and location of nevi, however, standardization of surgery and anesthesia is impossible. Rather, the type of anesthesia must be individually selected in view of the child’s age, the location of the nevi, the magnitude of the operation, the experience of the surgeon and the anesthesiologist, and the extent to which each of the two is involved in the surgical procedure. The options range from general anesthesia in intubation or spontaneous breathing through so-called “dual anesthesia“, i.e. brief general anesthesia with simultaneous conduction or infiltration anesthesia, to pure local anesthesia with or without premedication. Dose problems in relation to body weight have high priority in all these forms of anesthesia. Equally important in actual practice is good psychological guidance of the child in order to lessen his unavoidable perturbation when confronted with an unknown procedure. If contact with the child is good, middle-sized procedures in infiltration or conduction anesthesia are quite feasible.
