Topische Phospholipide bei kutanen Lymphomen

Zusammenfassung

15 Patienten, mit histologisch gesicherten kutanen Hautlymphomen, wurden topisch mit Hexadecylphosphocholin (Miltefosin^®) behandelt. Acht Patienten litten an T-Zell-Lymphomen, fünf an B-Zell-Lymphomen und zwei an einer lymphomatoiden Papulose. Miltefosin^®-Lösung wurde in der ersten Woche einmal täglich, ab der zweiten Woche zweimal täglich für 8 Wochen, in einem maximal 400 cm² großen Indikatorareal, appliziert. In diesem Indikatorareal ergaben sich bei den T-Zell-Lymphomen zweimal komplette Remissionen (CR), zweimal partielle Remissionen (PR), zweimal kein Ansprechen (NR) und eine Progression (PD). Einmal kam es zu einem vorzeitigen Abbruch. Bei den B-Zell-Lymphomen erzielten wir eine CR, drei PR und einmal NR. Bei der lymphomatoiden Papulose kam es in beiden Fällen zu einer CR. Systemische Nebenwirkungen traten in keinem Fall auf, die lokalen Nebenwirkungen beinhalteten Rötung, Schuppung und leichte Atrophie.

Summary

15 patients with histologically confirmed cutaneous lymphomas were treated with hexadecylphosphocholine oinment (Miltefosin). Eight patients suffered from T-cell lymphoma, five patients had B-cell lymphoma and two had lymphomatoid papulosis. During the 1st week Miltefosin was applied once daily, in the following 7 weeks twice daily, over an area of up to 400 cm². Among the cutaneous T-cell lymphomas two complete remissions (CR), two partial remissions (PR), two nonresponses (NR) and one progression of disease (PD) were observed. One patient was not evaluable. Among the five patients with B-cell lymphoma, one CR, three PR and one PD were seen. Therapy was tolerated without any systemic side effects. Local side effects included erythema, scaling and discrete atrophy.
