Zusammenfassung
Die am häufigsten angewendeten Operationsverfahren an der Wirbelsäule sind die Nukleotomie und die Spondylodese. Bei der Nukleotomie wird Bandscheibengewebe operativ entfernt, so daß eine Kompression des Nervs aufgehoben wird. Bei der Spondylodese kommt es zur Versteifung einzelner Wirbelsäulenabschnitte, indem benachbarte Bewegungssegmente untereinander fusioniert werden. Die in katamnestischen Untersuchungen erhobenen Erfolgsquoten variieren über einen weiten Bereich. Je nach Ausgangssituation werden in 50–80% der an Bandscheibe bzw. am Bewegungssegment operierten Patienten zufriedenstellende Resultate erzielt. Um die Erfolgsquoten zu steigern, wurde beispielsweise der Versuch unternommen, die Indikationsstellung durch eine Verfeinerung der neurologischen und orthopädischen Diagnostik zu präzisieren. In diesem Kontext wurden auch psychologische Variablen erhoben. Zunächst erlangte der MMPI große Popularität, da gefunden wurde, daß sich erfolgreich von erfolglos operierten Patienten hinsichtlich der „neurotischen Triade“ unterschieden. Da aber die relevanten Skalen des MMPI auf belastungsinduzierte körperliche Symptome hinweisen und da Fortschritte in der Erforschung der Chronifizierung von Akutschmerz erzielt wurden, traten schmerzspezifische Untersuchungsverfahren immer stärker in den Vordergrund. Zum Abschluß werden die Ergebnisse einer eigenen Studie an Nukleo- tomiepatienten berichtet, deren Untersuchungshypothesen aus Modellen zur Chronifizierung von Akutschmerz abgeleitet wurden.
Summary
Surgical procedures most often applied to the spine are nucleotomy and spondylodesis. In nucleotomy, tissue of the disk is removed in order to reverse compression of a spinal nerve. In spondylodesis, a partial fixation of the spine is attained by fusing adjoining segments. Success ratios of these procedures reported in follow-up assessments vary over a vast range. Depending on the initial situation, 50%–80% of the patients who have been operated on the spine show a beneficial outcome. One way to improve outcome ratios was the reevaluation of the neurological and orthopedic assessment procedures applied, taking also psychological variables into consideration. At the beginning of this line of research, the MMPI became very popular, because of a differentiation between treatment successes and treatment failures enabled by the combination of some of the scales (“neurotic triad”). Recently, there was a shift to assessment procedures more specific to chronic pain, especially when the critical MMPI profile was found to be an indicator of a physical stress response rather than a neurotic personality trait. Pain-specific assessment instruments are based on assumptions about the process of chronicity in pain that have found widespread acceptence in behavioral medicine. Studies being conducted in the behavioral context — author research included — are presented.
