Psychologische Interventionen bei entzündlichen Gelenkerkrankungen

Zusammenfassung

Studien mit Patienten, die an entzündlichen Gelenkerkrankungen leiden, belegen, daß die Erkrankung eine starke Belastung darstellt und daß viele Patienten einer Unterstützung bei der Schmerz- und Krankheitsverarbeitung bedürfen. Untersucht wird die Effektivität von Patientenedukation, Biofeedback und einem Training zur Schmerz-und Krankheitsbewältigung (SBT). Bei Patienten mit chronischer Polyarthritis (cP) wurden positive Effekte der Edukation und des SBT nachgewiesen, während die Wirksamkeit des Biofeedback fraglich ist. Sowohl bei cP-Patienten als auch bei Patienten mit ankylosierender Spondylitis wirkt sich SBT stärker auf eine emotionale Stabilisierung als auf das Schmerzerleben aus.

Summary

Studies conducted in a behavioral context focus on coping strategies and give support to the assumption that inflammatory rheumatic diseases exert a high degree of distress on the patients and that patients suffering from rheumatoid arthritis (RA) or from ankylosing spondylitis (AS) are in urgent need of psychological support. Behavioral research includes the topics of patient education, biofeedback, and training procedures for the promotion of coping with chronic pain and disease. In RA patients, a beneficial outcome of education and stress coping procedures was demonstrated, whereas there still is considerable doubt about the effectiveness of biofeedback training. Both in RA and AS patients, the promotion of coping seems to have more effect on the stabilization of the emotional state than on reducing pain intensity.
