The Whipple Pancreaticoduodenectomy for Pancreatic Cancer

Abstract

Whipple first described pancreaticoduodenal resection in 1935, for a cancer of the ampulla of Vater [1]. Within a short time, the operation was being performed for carcinoma of the head of the pancreas, although it underwent a number of modifications before the one-stage operation that we know today was developed [2, 3]. By 1953, Cattell and Warren reported that they had performed 102 Whipple procedures at the Lahey Clinic [4]. Of these, 88 were for malignant disease of the periampullary area, and 46 of the 88 were for carcinoma of the head of the pancreas. Nineteen of the 46 were done as one stage resections, the others had some form of biliary decompression performed as an initial procedure. The mortality rate in this early series of 46 patients was 17.3% (eight deaths).
