Zusammenfassung

Herzchirurgische Eingriffe unter extracorporaler Zirkulation induzieren eine deutliche Dysregulation der Immunabwehr, insbesondere der spezifischen zellvermittelten Immunität (CMI), und parallel dazu kommt es zu einer systemischen, hyperinflammatorischen Immu- nantwortreaktion. Diese Immunantwort nennen wir Akutphasenreaktion, sie wird vermittelt durch eine Reihe von Mediatorsystemen, insbesondere durch die proinflammatorische Cytokincascade (Interleukin-1 (IL-1), Tumornekrosefaktor α (TNF-α), Interleukin-6 (IL- 6)), und ist in ihrer Intensität proportional zum Traumaausmaß. Die Dysregulation der CMI nach mechanischem Trauma wird repräsentiert durch die Dissoziierung der integrierten Monocyten (MO)/T-Lymphocyten Interaktion [1].
