Zusammenfassung

Das Verfahren der perkutanen transluminalen Angioplastie von arteriosklerotischen Läsionen geht auf Charles Dotter und Mel Judkins zurück [72]. Diese perkutanen Behandlungen wurden zunächst nicht mit einem Ballonkatheter, sonder mit teleskopartigen, koaxialen Kathetersystemen durchgeführt. Nach Passage der Stenosen oder Verschlüsse mit einem Führungsdraht wurde das betroffene Segment zunächst durch einen 2,6 mm (entspricht 8 French) im Durchmesser betragenden, geraden Katheter gedehnt. Über diesen 8-F-Katheter wurde anschließend zur weiteren Dilatation ein 12-F-Katheter vorgeschoben. Durch dieses Verfahren war eine maximale Aufdehnung des Gefäßinnenlumens auf 4 mm möglich [298]. Die perkutane Gefäßrekanalisation wurde in den USA zunächst mit Zurückhaltung aufgenommen und Charles Dotter von einigen Kollegen als „crazy Charlie“ tituliert. Van Andel und Zeitler verbesserten das ursprüngliche Verfahren durch die Entwicklung eines an der Spitze schmalen, nach proximal stufenlos breiter werdenden Katheters, wodurch ein weniger traumatisierendes Vorgehen ohne Katheterwechsel möglich wurde.
