Zusammenfassung
Der Zusammenhang zwischen diätetischer Kochsalzaufnahme und Blutdrucksteigerung ist bereits in den frühen Schriften der chinesischen Medizin dargestellt. Auch in den Hypothesen des 20. Jahrhunderts scheint dem Natriumchlorid eine wesentliche Rolle in der Pathogenese der essentiellen Hypertonie zuzukommen Über welchen Mechanismus jedoch die diätetische Kochsalzaufnahme den Blutdruck beeinflußt, ist noch ungeklärt. Nur bei einem Teil der hypertensiven Patienten kommt es nach erhöhter Kochsalzeinnahme zu einer Blutdrucksteigerung. Verschiedene Hypothesen sind in den vergangenen Jahrzehnten postuliert worden, die diese Kochsalz-bedingte Blutdrucksteigerung erklären sollten. Eine der wesentlichen Hypothesen zur Pathogenese der essentiellen Hypertonie hat die orale Kochsalzaufnahme mit einer Natriumchlorid-abhängigen Störung des zellulären Calciumtransports in Zusammenhang gebracht. Diese, von de Wardener und MacGregor postulierte und von Blaustein modifizierte Hypothese verknüpft die Ausscheidungsstörung von NaC1 bei Patienten mit essentieller Hypertonie mit dem Anstieg der intrazellulären Calciumkonzentration in den glatten Muskelzellen dieser Patienten (Abb. 1). Es kommt nach dieser Hypothese durch überschießende Kochsalzzufuhr zu der gesteigerten Sekretion eines vermutlich hypothalamischen Faktors, welcher eine Hemmung der Natrium-Kalium-ATPase an der Zellmembran bewirkt. Die Hemmung dieses Enzyms führt dann zu einem Anstieg der intrazellulären Natriumkonzentration in der Zelle. Dies beeinflußt den zellulären Natrium-CalciumAustauschmechanismus. In den glatten Muskelzellen der Gefäße könnte es über diesen Mechanismus zu einer Erhöhung der intrazellulären freien Calciumkonzentration und damit zu einer gesteigerten Kontraktion der Gefäße kommen.
