Zusammenfassung
Die medikamentöse Therapie der Epilepsien und das Vorgehen bei Problemfällen läßt sich in 4 Schritte gliedern (Abb. 1). Der 1. Schritt nach Stellung der Behandlungsindikation ist die medikamentöse Regelbehandlung, die im Einsatz von Antiepileptika der 1. Wahl in Monotherapie, notfalls auch in Kombinationstherapie besteht. Bei unzureichendem Behandlungserfolg können als 2. Schritt Antiepileptika der 2. und 3. Wahl und bereits im Handel befindliche Medikamente mit anderer Hauptindikation eingesetzt werden. Der 3. Schritt besteht im Einsatz neuer, noch nicht im Handel befindlicher Antiepileptika, der 4. Schritt in nichtmedikamentösen Therapieverfahren, insbesondere dem operativen Vorgehen. Möglicherweise ergibt sich in der Zukunft bei einem Teil der Fälle eine andere Reihenfolge, die von I direkt nach III oder IV führt (Abb. 1).
