Vigabatrin bei Epilepsien des Kindesalters

Zusammenfassung

Vigabatrin (γ-vinyl, γ-Aminobuttersäure), ein spezifischer Hemmstoff der GABA-Transaminase, hat sich als ein wirksames Antiepileptikum zur Behandlung von komplexen fokalen Anfällen im Erwachsenenalter herausgestellt (Rimmer u. Richens 1984; Gram et al. 1985; Loiseau et al. 1986; Tartara et al. 1986; Browne et al. 1987; Tassinari et al. 1987). Fast 50% der Patienten haben eine mindestens 50%ige Abnahme der Anfallshäufigkeit unter der Behandlung mit Vigabatrin. Patienten mit generalisierten Anfällen wiesen hingegen keine signifikante Wirkung auf (Michellucci et al. 1989).Untersuchungen zur Langzeitwirkung zeigten, daß die meisten Patienten mit einem guten initialen Ansprechen auf das Medikament diese Wirkung während einer mittleren Nachuntersuchungsperiode von 22–23 Monaten aufrechterhielten (Browne et al. 1989; Tartara et al. 1989; Remy u. Beaumont 1989). Obwohl pharmakokinetische Studien eine kurze Halbwertszeit im Blut von 5–7 h nachwiesen (Schechter 1989), bleibt die Wirkung auch bei einer Tagesdosis erhalten (Ben-Menachem et al. 1989). Eine signifikante Abnahme der Phenytoinplasmakonzentration um 23%, die klinische Auswirkungen haben könnte, ist die einzige Arzneimittelwechselwirkung mit anderen Antiepileptika, die derzeit bekannt ist (Rimmer u. Richens 1984).
