Zusammenfassung
Vigabatrin hat sich als wirksames Antiepileptikum erwiesen (Gram 1988; Rimmer et al. 1987). Der therapeutische Einsatz von Vigabatrin war durch die im Tierexperiment an einzelnen Spezies beobachtete Vakuolenbildung im Marklager der Großhirnhemisphären zunächst in Frage gestellt, doch ließen sich ähnliche Veränderungen bei Primaten nicht nachweisen. Im klinischen Einsatz erwies sich Vigabatrin dann als relativ nebenwirkungsarm und wirksam und fand somit bereits jetzt eine breite Anwendung (Ben-Menachem et al. 1990; Browne et al. 1989; Mumford und Dam 1989; Tartara et al. 1989; Treiman 1989; Remy und Beaumont 1989; Tassinari et al. 1987; Dam 1989; Remy et al. 1989). Neben klinischen Untersuchungen, laborchemischen Bestimmungen und CT/MRT-Kontrollen wurden vereinzelt auch Verlaufsuntersuchungen mittels evozierter Potentiale durchgeführt, um mögliche zerebrale Funktionsstörungen frühzeitig erfassen zu können (Cosi et al. 1988, 1989; Liegeois-Chauvel et al. 1989 a, b; Hammond et al. 1988). Daneben wurden Gewebeproben von zuvor mit Vigabatrin behandelten Patienten, die bei Operationen oder post mortem gewonnen wurden, histologisch untersucht (Agosti et al. 1990; Pedersen et al. 1987). Mit beiden Methoden konnten keine Vakuolenbildungen im menschlichen Nervensystem nachgewiesen bzw. Funktionsstörungen aufgezeigt werden.
