Operationsverfahren bei Glaukom und Katarakt

Zusammenfassung

Bei der Behandlung von Patienten mit einer operationswürdigen Katarakt und gleichzeitigem Glaukom bestehen grundsätzlich drei Möglichkeiten des opera-tiven Vorgehens: 1. Kataraktextraktion alleine;2. Glaukomfiltrationsoperation alleine, gefolgt vonder Kataraktextraktion zu einem späteren Zeitpunkt;3. kombinierte Katarakt- und Glaukomoperation in einem Eingriff. Die kombinierten Operationsverfahren haben ein geringfügig größeres Komplikations-profil als die alleinige Kataraktoperation wie auch einegeringere Prognose der Druckregulierung verglichenmit einer alleinigen Filtrationsoperation. Aus diesen Gründen sollte der Operateur jeweils alle drei Möglichkeiten in Betracht ziehen und die für den individu-ellen Patienten geeignetste wählen. Es sollen in diesem Kapitel zunächst die relativen Indikationen für diesedrei Möglichkeiten besprochen und dann die speziel-len chirurgischen Techniken auf gezeigt werden.
