Zusammenfassung
Nach den ausführlichen Untersuchungen insbesondere der Arbeitsgruppe von Goldmann [7–9] sind die präoperativ feststellbaren Risikofaktoren bekannt, die die perioperative Phase negativ beeinflussen. Wenn eine koronare Herzerkrankung, eine manifeste Herzinsuffiziens, eine Aortenstenose oder Arrhythmien vorliegen, ist von einer erhöhten perioperativen Komplikationsrate und Letalität auszugehen (Tabelle 1). Dementsprechend sind viele Versuche unternommen worden, im Rahmen einer internistischen präoperativen Untersuchung die Risikofaktoren aufzudecken und entsprechend zu behandeln, die bei einer Abdominal- oder Thoraxoperation für die auftretenden Komplikationen verantwortlich sind. Im folgenden soll aus internistisch-kardiologischer Sicht auf 2 Problemkreise, die als Hauptrisikofaktoren bei Patienten über 49 Jahre für die Operationsletalität anzusehen sind, näher eingegangen werden.
