Zusammenfassung
Eine wesentliche Aufgabe der Narkoseführung bei herzkranken Patienten ist die Aufrechterhaltung der Kreislaufstabilität. Darunter verstehen wir die Vermeidung sowohl einer Herz-Kreislauf-Depression als auch hyperdynamer Entgleisungen. Das myokardiale Gleichgewicht zwischen O2-Angebot und -Verbrauch wird durch folgende Faktoren beeinflußt:

Tachykardie, erhöhte Vor- und Nachlast und erhöhte Kontraktilität steigern den myokardialen O2-Verbrauch.

Tachykardie, erhöhte Vorlast und Hypotension reduzieren das myokardiale O2-Angebot.

Abnahme des Herzzeitvolumens führt außerdem zur Minderversorgung der Organe und zur gemischtvenösen und schließlich arteriellen Hypoxämie.
