Störungen der monoaminergen Neurotransmittersysteme bei Demenz vom Alzheimer-Typ

Zusammenfassung

Aus verschiedenen Untersuchungen ergeben sich Hinweise auf zerebrale neurochemische Veränderungen bei Patienten mit Alzheimer-Krankheit (AD) und seniler Demenz vom Alzheimer-Typ (SDAT). Die meisten Studien gehen davon aus, daß es sich um Neurotransmitterstörungen in der grauen Hirnsubstanz handelt. Von Veränderungen in der weißen Hirnsubstanz wurde jedoch ebenfalls berichtet. Brun u. Gustafson [7] beschrieben degenerative Veränderungen der weißen Hirnsubstanz dementer Patienten. Es handelte sich dabei nicht nur um Patienten mit zerebralen Durchblutungsstörungen, sondern auch um Patienten mit AD/SDAT. Neurochemische Untersuchungen der Bestandteile weißer Hirnsubstanz haben ebenfalls gezeigt, daß die Myelinanteile bei AD und SD AT geringere Konzentrationen aufweisen [8, 13, 20]. Veränderungen der weißen Hirnsubstanz werden als inkomplette Infarkte beschrieben, ihr Wesen ist jedoch noch nicht vollständig geklärt. Auch ist nicht bekannt, in welchem Zusammenhang diese Störungen mit dem Demenzsyndrom stehen. Auszuschließen ist jedoch, daß die Veränderungen der weißen Hirnsubstanz lediglich als Sekundärerscheinungen neben den Veränderungen der grauen Hirnsubstanz auftreten.
